Das Frasier-Syndrom (FS) ist eine sehr seltene angeborene Erkrankung mit den Hauptmerkmalen Pseudohermaphroditismus und fortschreitende Glomerulopathie.cite-ref-orpha-1-0[1]

Die Bezeichnung bezieht sich auf den Erstautoren der Erstbeschreibung aus dem Jahre 1964 durch die US-amerikanischen PΓ€diater S. D. Frasier, R. A. Bashore und H. D. Mosier.cite-ref-2[2]

Verbreitung

Die HΓ€ufigkeit wird mit unter 1 zu 1.000.000 angegeben, bislang wurden etwa 50 Betroffene beschrieben. Die Vererbung erfolgt autosomal-dominant.cite-ref-orpha-1-1[1]

Ursache

Der Erkrankung liegen Mutationen im WT1-Gen auf Chromosom 11 Genort p13 zugrunde, welches fΓΌr ein DNA-bindendes Zinkfingerprotein kodiert. Das Wilms-Tumor-Protein ist gleichfalls beteiligt.cite-ref-3[3]

Dieses Gen ist auch beim Denys-Drash-Syndrom, beim Meacham-Syndrom und der Hypospadie beteiligt.cite-ref-4[4]

Klinische Erscheinungen

Klinische Kriterien sind:cite-ref-orpha-1-2[1]

β€’ mΓ€nnlicher Pseudohermaphroditismus, normal weibliches Γ€ußeres Genitale, Stranggonaden und einen Karyotyp 46,XY.
β€’ glomerulΓ€re Nephropathie mit Proteinurie und nephrotischem Syndrom
β€’ als junge Erwachsene terminales Nierenversagen
β€’ deutlich erhΓΆhtes Risiko fΓΌr die Entwicklung eines Gonadoblastomes

Klassifikation

Nach dem PhÀnotyp der Àußeren Genitalia wurde folgende Einteilung vorgeschlagen:cite-ref-5[5]

β€’ Typ 1 mit weiblichem Genitale, Karyotyp 46,XY, hΓ€ufigste Form
β€’ Typ 2 mit mΓ€nnlichem Genitale, Karyotyp 46,XY
β€’ Typ 3 mit weiblichem Genitale Karyotyp 46,XX

Bei letzterem Typ wurden keine Gonadoblastome beobachtet.

Differentialdiagnose

Abzugrenzen sind das Swyer-Syndrom und das Denys-Drash-Syndrom.cite-ref-orpha-1-3[1]
